What causes my 96 explorer od light to blink and hard shift in and out of 2nd gear?

only happens at about 20 miles into a road trip and seems to be triggered by the heavy acceleration onto the highway. turning off and restarting the engine resets the od light and the clunking on 2nd gear goes away after a few days of intown driving
